Why?
In order to have the most accurate information and to be able to provide the best possible service to you, a review of your child's school records is necessary. This process also helps get your records and paperwork organized, if it isn't already.

Why?
Initial Intake Form:
This form is a secondary method of collecting historical information that is not part of the school record collection. It is of specific set of questions designed with regard to your child's educational history. Often times these questions help to jog your memory of critical information, or bring forth information that you may have previously disregarded as unimportant.
IEP Meeting Form:
This form is designed to help prepare for an IEP meeting by collecting
Transition:
This form is designed to collect important information with regard to planning for you child's transition from high school to either post secondary school or into community living.

Narrative Chronological History
Why?
From there a chronological narrative is written with your input and flexibility to review and edit. The narrative is a fact based chronological narrative history of events leading up to the current issue(s). Should resolution not be reached, with due process being the next step all information is now in an unemotional format ready for presentation.
